Issue not being saved

A thousand apologies if this is already answered ; I did some preliminary searches and could not find an answer.

I installed the Issue tracker component, under permissions I explicitly allowed create and edit own for "Registered". After this I created a "Create an Issue" menu item, then I created a regular joomla user. When I log in as this user from the front-end, I can access the 'Create an issue' page, - when I save my issue, It redirects to the index (can I change this?) without a message. Navigating to any other page shows me a message like the following:
Message
Issue No: Z4SXE55J2Y
Issue Saved!

Which seems great! ... except when I look at the issue log on the back-end, there are no issues in the listing.

Any ideas?

Sounds like there is an error occuring which is not being picked up, and the message is then displayed. Any error messages shown if you have system debug on?

Also details of the system configuration would help if possible?
